NATURE AND SCOPE

Psychiatric nurses work with clients whose primary care needs relate to mental and developmental health. Psychiatric nurses recognize the complex relationship between emotional, developmental, physical and mental health, as well as the role that social factors, culture and spirituality play in illness and recovery.
RPNs work directly with the medical team and with interdisciplinary teams within the TRC. They need to continually exercise sound judgement in making decisions and solving problems to meet clinical challenges and the health care needs of patients/ clients. With their specific education and experience, RPNs provide crisis and treatment intervention on a daily basis. They must possess excellent interpersonal and communication skills.
The RPN will work directly with inpatient addiction treatment and management clients in collaboration with the TRC medical team.

RESPONSIBILITIES AND DUTIES

• Conducting initial intake, nursing assessment and triage level of required care by the responsible provider.
• Advanced Psychiatric evaluations for new patients and existing program patients.
• Assessing patients daily for level of physical, emotional, and social stability
• Offering guidance throughout treatment for better goal attainment
• Monitoring for change in mood or demeanor, recognition of behavior patterns, behavior changes and communicating with the interdisciplinary team at Thorpe.
• Develops care plans and communicates with the interdisciplinary team.

• Intervening with de-escalation techniques to lead the patient back to safety and stability.
• Specializes in crisis management and safety plans while working with the
medical team.
• Providing support to Community Support Workers (CSWs) as appropriate
• Provides direct nursing care to patients with a variety of primary care needs including all health complexities with recognition of when it is appropriate to contact the NP/Physician.
• Demonstrate the ability to operate health equipment safely.

Intake Coordinator

INTAKE COORDINATOR

NATURE AND SCOPE

Reporting to the Community Support Manager, the Intake Coordinator is responsible for the direct delivery of intake services to service users effectively and respectfully with a wide range of individuals of different ages, cultural background, ethnicity, and orientation, facilitating a positive experience into Detox, and Residential service. Strong moral and ethical reasoning, resourcefulness, adaptability, and capable of diffusing emotionally charged situations or potentially volatile interactions is key and having compassionate understanding for individuals transitioning into/out of treatment.

RESPONSIBILITIES AND DUTIES

Program Intake
Welcomes service users and family members to TRC; facilitates and coordinates the intake process.
• Coordinates all relevant service user communication with necessary staff.
• Ensures the admission process is conducted in a timely and proficient manner.
• Documenting and safely disposing of contraband items (drugs, paraphernalia, weapons, etc.) and securing unapproved items (phone, keys, perfumes etc.).
• Review consent and the limits of confidentiality and complete the consent to release/retrieve information form(s) and upload required forms to internal client database(s).
• Coordinates luggage checks, building tours and service user orientations.
• Introduces the service user to assigned program staff.
• Support the service user to settle into their room.
• Ensures intake paperwork is accurately completed and maintained in accordance with FOIP, agency standards and the Health Information Act.

Service User Discharge
• Maintains statistics for service users leaving treatment and program.
• Ensures service users belongings have been returned to individuals following treatment.
• Ensures service user completes all discharge paperwork.

Administrative Reporting
• Provides effective documentation for service user in accordance with agency standards.
• Maintains adequate communication with the clinical team and related staff members.
• Inputs complete and accurate documentation on internal client database(s).
• Relays appropriate communication to necessary service users and staff members.
• Provide relief and support to CSW team where necessary.

Health Care Aide

HEALTH CARE AIDE

NATURE AND SCOPE

Reporting to the Medical Manager or designate the Health Care Aides (also known as HCAs) are valued members of the TRC team and provide support to the patients and staff. HCA’s provide comfort and support to those in the inpatient addiction treatment setting. They may assist with personal hygiene, recreational activities and emotional support to the patients. Additionally, they support varied staff roles within the TRC including nursing staff and the Community Support Workers (CSW) with a goal of a supportive and therapeutic resource for admissions, discharges and transportation as required.
HCA’s document and report on their observations and communication concerns to nurses and other licensed health care professional and in the TRC EMR

RESPONSIBILITIES AND DUTIES

• Demonstrate an understanding of the patient care plan and the role of the HCA in implementing the care plan.
• Encourage and support the patient’s efforts to maintain and/or enhance their health, wellness, independence and quality of life.
• Promote the patient’s ability to guide and participate in their own care to the greatest degree possible. In collaboration with the interdisciplinary team at TRC.

DETOX MENTOR

NATURE AND SCOPE

Under the general supervision of the Clinical Director or designate, the Detox Mentor functions as a member of the multi-disciplinary care team to provide services that promote wellness while empowering individuals to achieve their highest level of functioning. The Detox Mentor provides client focused treatment to clients through assessment and treatment planning, individual and group support as well as lecture presentations.

MENTOR
• Provides one-on-one guidance to the clients in the Detox program.

• Assess and provide direct intervention or referral to appropriate service providers/programs.

• Vacuums Carpets: Thoroughly vacuums all carpeted surfaces to maintain cleanliness and remove dust and debris.

Councellor

COUNSELLOR

NATURE AND SCOPE

The Counsellor functions as a member of the multi-disciplinary care team to provide services that promote wellness while empowering individuals to achieve their highest level of functioning. In delivering these services, the Counsellor utilizes a case management approach, consulting with their clinical team and other supporting agencies. The Counsellor is required to provide coordination in the areas of case management, case planning, and implementation of programming. The Counsellor provides support to program offerings including (but not limited to): Detox, Transition, and Residential Programs.
The Counsellor main job is to assist a patient along the continuum of care from admission to Continuing Care. Counsellors at Thorpe provide one on one therapy and group processes that contribute insightful, practical and meaningful information to the development of treatment plans. Each Counsellor carries a caseload and provides clinical treatment in the form of, but not limited to, individual, family and group counseling daily. As a member of the treatment team, the Counsellor meets weekly in collaborative fashion with the rest of the team to discuss their patient’s progress. Works with an interdisciplinary team to develop a holistic treatment process and plan. Possesses the ability to offer trauma informed care and culturally responsive counselling.
A Counsellor must possess foundational knowledge of concurrent disorders, process addiction and thorough understanding of substance use disorder and Recovery Orientated Systems of Care.

RESPONSIBILITIES AND DUTIES

• Meets with residents within 24 hours of counsellor assignment to establish therapeutic rapport.
• Develop a treatment plan for each client based on the assessment of presenting issues.
• Completes patient orientation, including a biopsychosocial assessment within a week of a patient being medically cleared.
• Adapt treatment modalities to the client’s individual characteristics and needs.
• Explore all available resources in maximizing the most effective plan for the client.
• Utilize an interdisciplinary approach to treatment where appropriate.

Community Support Worker

COMMUNITY SUPPORT WORKER

NATURE AND SCOPE

Reporting to the CSW Manager or designate, the Community Support Worker provides support to the clients and staff at TRC. This position promotes active recovery by engaging clients in recreation, activities and provides transportation for clients to off-site meetings and events. In coordination with the clinical care team, the Community Support Worker works with administration, coworkers, and other agencies to support TRC’s commitment in order to maximize recovery and client’s engagement throughout the recovery process.

RESPONSIBILITIES AND DUTIES

RECREATION FACILITATION
• Organizes, plans, and facilitates structured and unstructured activities both on and off site to provide client engagement specific to their needs.
• Facilitates scheduled activities including meditations, walks, recreational activities, etc.
• Transports clients to appropriate meetings, appointments, activities, and events.
• Assists with the organization and facilitation for internal and external events including bookings, movies, lectures, and program offerings.

CLIENT INTERACTION
• Welcomes clients and family members to TRC; provides agency information and follows up with questions, inquiries, and concerns.
• Communicates program schedules and daily activities to clients.

NATURE AND SCOPE

The Continuing Care Coordinator is responsible for the onboarding and coordination of continuing care programming to clients of Thorpe Recovery Centre. The Continuing Care Coordinator will work with the clinical team to set clients up with continuing care support and create continuing care plans for each client.

RESPONSIBILITIES AND DUTIES

CLIENT SERVICES
• Ensure a supportive & positive experience for clients, referral sources and other external providers.
• Communicate regularly with referral providers about new clients and ensure that the external sources have all necessary information about TRC.
• Work directly with clients, families, and external partners to understand the needs and preferences of clients.
• Connect regularly with clients as their discharge date approaches to ensure they are aware of their continuing care plan and familiar with other resources available.
• Using a variety of tools, assess clients’ needs, situations, strengths and support networks to determine their goals and develop plans to meet identified goals.
• Research, refer, and advocate for clients to support them in accessing community resources that would improve their well-being, including but not limited to connections to sober living, a recovery sponsor, support groups, and an addictions counsellor.
• Supports Clinical Admin to work with clients work with government agencies to apply for and receive benefits such as EI and Income Support (where applicable – clinical admin role)
• Update team members with any information that might require attention or intervention.
• Make outpatient arrangements and arrange transportation.
• Facilitate calls with drug court, lawyers, and Child and Family Services.
• Co-facilitate/facilitate group sessions.
• Offer support during the transition from detox to treatment, help explore treatment options, complete admission paperwork, secure safe temporary housing, and arrange appointments with an addiction’s counsellor.
• Help clients engage in volunteer work as determined by the program schedule.

ADMINISTRATION
• Monitor discharge dates for all TRC clients and ensure their continuing care plans are well established.
• Support the clinical team by ensuring all client files are kept up to date with continuing care plans and progress.
• Ensure all discharge plans are being followed and connecting with the clinical team with any changes.
• Manage client case files, maintaining full documentation of treatments, service providers, case details, and client progress.
• Make behavioral observations, mental status assessments, psychosocial interventions, and documentation.

• Bachelor’s or master’s degree in social work (or working towards)
• Preference will be given to those with a Social Services Certificate or Diploma.
• Active or eligible for registration with the provincial social work body (ACSW/SASW).
